Group Life Benefit
From 1 times up to 5 times annual salary (Maximum $1 500 000)
Re-instatement of the Group Life Benefit option
A re-instatement option is available for a group scheme with Group Life Benefits as well as Critical Condition Benefit. Should a Member survive 31 days after a Critical Condition Incident the original Group Life Benefit will continue to apply.
Accidental Group Life Benefits
Provide for an additional lump sum, if the Member’s death is a result of an accident.
The benefit is a multiple of the Member’s annual salary, but limited to US$ 600 000 or 100% of the Group Life Benefit, whichever is lower.
This benefit is only available when the Group Life Benefit is selected, and cannot be higher than the Group Life Benefit.
Spouse's Group Life Benefits
Compensate the Member on the death of the spouse.
The benefit is a multiple of the Member’s annual salary, but limited to US$ 600 000 or 2 times the member's annual salary, whichever is lower.
This benefit is only available when the Group Life Benefit is selected, and cannot be higher than the Group Life Benefit.
Payment of benefits
Death and Accidental Benefits are paid according to the Member’s nomination of beneficiaries’ instructions.
The Spouse’s Group Life Benefit is payable directly to the group scheme Member.
Eligibility
The maximum age for a Member to join the group scheme is 64.
A group scheme Member is allowed to enjoy Group Life Benefits up to a maximum age of 70.
Exclusions
Exclusions applicable to all the benefits are:
Participation in war, riot and civil commotion, terrorism,
all claims related to atomic, biological and chemical warfare, or terrorism.
Conversion Option
The option allows a Member to convert his Group benefits to an Individual life Policy, underwritten by Global Alliance Life, at individual life rates subject to medical underwriting.
If the Member resigns from the Employer’s service, before age 60, he is eligible for this option, but needs to apply to Global Alliance Life within 30 days of resignation.
The Employer can only select group scheme Conversion Option benefits at the commencement date.
Requirements:
HIV/Aids test.
Cotinine test.
If a member dies during the 30 day period, after becoming eligible to exercise the Conversion Option, he will automatically qualify for the full benefit that he would have been eligible to convert, subject to meeting the group scheme Policy terms and conditions.

Claims Process
Notification period
Within 3 months from the date of the incident giving rise to a claim.
Submission of claim documents
6 months from the date of death. Any additional documentation must be submitted within 60 days of the request by Global Alliance Life.
Claim documents to be submitted for a Death claim:
- A completed notification of death claim form,
- A completed signed and stamped death claim form,
- an original certified copy of the Member’s Identification document or passport,
- an original certified copy of the Member’s official death certificate,
- an original certified copy of the Member’s last payslip,
- an original certified copy of the Beneficiary’s Identification document / passport or birth certificate,
- a beneficiary nomination form or an Employer resolution for the distribution of the benefits to the beneficiaries,
- an original certified copy of the Member’s latest payslip,
- proof of membership
- an original certified copy of the beneficiary bank details on a bank letterhead, and
- any other documentation requested by the company necessary to expedite the payment of the claim.
Claim documents to be submitted for a Spouse’s claim:
- A completed death claim form,
- an original certified copy of the identification document or passport of the member,
- an original certified copy of the identification document of passport of the member’s Spouse
- an original certified copy of the official death certificate,
- an original certified copy of the member’s last payslip,
- an original certified copy of the member’s bank details on a bank letterhead, and
- any other documentation requested by the company necessary to expedite the payment of the claim.
Claim documents to be submitted for an Accidental Death claim:
- A completed death claim form,
- an original certified copy of the identification document or passport,
- an original certified copy of the official death certificate,
